Giant Pancreatic Pseudocysts: Procedures in Management

چکیده
The effect of size of giant pancreatic pseudocysts on the outcome of internal drainage procedures. Surgery Gynecology & Obstetrics 173, 111m174. Pancreatic pseudocysts (PP) that fail to resolve spontaneously are optimally treated by internal drainage to a viscus. Pseudocysts adherent to the stomach are usually drained by way of cystgastrostomy. Recent experience with giant pseudocysts > 15 centimeters), however, challenges this approach. Fifty-two patients with pancreatic pseudocysts of various sizes were treated from 1982 to 1986 at the Massachusetts General Hospital. Twenty-eight PP were suitable for internal drainage. The posto-perative complication rate was directly proportional to the size of the pseudocyst. Four patients had giant PP, three of which occurred after an attack of acute pancreatitis. All four were treated by cystgastrostomy. Three of four patients with giant pseudocysts had life-threatening postoperative complications as a result of incomplete emptying of the cyst, and two patients died. No evidence of anastomotic leakage could be demonstrated by upper gastrointestinal series or computed tomographic scans. Transgastric drainage tubes in these three instances were not protective. We conclude that cystgastrostomy may not be appropriate for the treatment of giant pancreatic pseudocysts because it fails to provide dependent drainage of a large cyst cavity. If internal drainage is performed, the cyst should be anastomosed to a defunctio-nalized loop of jejunum in a dependent position. In some instances, external drainage of giant pancreatic pseudocysts may be safer than cystgastrostomy.

Surgical intervention for mediastinal pancreatic pseudocysts. A case series and review of the literature.
CONTEXT Development of mediastinal pancreatic pseudocysts is a rare complication of pancreatitis. There is currently no consensus on the optimal management of this condition, options for which include conservative management with somatostatin analogues, endoscopic drainage procedures and surgery. CASE REPORT Here we present two patients with mediastinal pancreatic pseudocysts which were initi...
متن کاملInternal drainage of giant acute pseudocysts: the role of video-assisted pancreatic necrosectomy.
BACKGROUND Internal drainage of giant pancreatic pseudocysts secondary to acute pancreatitis is frequently complicated with postoperative retroperitoneal infection and hemorrhage. Recent data suggest that the risk factor is unrecognized pancreatic necrosis; presumably, pancreatic necrosis becomes infected with bacteria introduced by the cystoenteric anastomosis. HYPOTHESIS Video-assisted panc...
متن کاملManagement of pancreatic pseudocysts—A retrospective analysis
BACKGROUND Pancreatic pseudocysts arise mostly in patients with alcohol induced chronic pancreatitis causing various symptoms and complications. However, data on the optimal management are rare. To address this problem, we analysed patients with pancreatic pseudocysts treated at our clinic retrospectively. METHODS We searched our clinical database for the diagnosis pancreatitis from 2004 till...

The outcome for endoscopic management of pancreatic fluid collections is dependent on the type of collection being treated. While pseudocysts have an excellent treatment response, the outcomes are modest for walled ‐off pancreatic necrosis. Recent advances in cross‐sectional body imaging have enabled more accurate distinction of pancreatic fluid collections, which in turn facilitates correct tr...
